MINOR RELEASE FORM - 17YRS & UNDER

If you register online, you will complete Minor Waiver as part of your registration. No other waivers are needed.

If you do not register online and are NOT coming to the event with your Minor Child, you will need to complete the Unaccompanied   Minor Waiver below.

UNACCOMPANIED MINORS (17yrs & Under)

All minors attending this event WITHOUT PARENTS , that did no complete the online waiver, must come with a notarized statement  signed by both  parents or legal guardian giving permission for another adult to act as guardian to sign releases.
